Not required for:
- First license of a brand-new vehicle
- Land vehicles

- Passenger Transportation License (for Omni buses only)

Payment:
- The revenue license will be issued upon completion of required payments with the above documents.

Tree Cutting Permits
Client’s Responsibilities:
- Filling out the first page of an application form correctly
- The copy of the deed of the land where the trees are situated and the copy of the plan of the land should be certificated by the Grama Niladhari and submitted along with the application.
- If there are any co-owners or other partners, their consent must be expressed and signed consent letters must be submitted before the Grama Niladhari. If deceased, submit the relevant death certificate.
- If the land is undivided, the consent of the other parties must be obtained in writing.
- In case of mortgaged land, written consent must be obtained from the relevant financial institution.
- The following documents must be submitted depending on the reason for requesting tree felling.
- Tree death, falling forward, failure to bear fruit, disease – Report of the Agricultural Advisor
- Cutting pursuant to a court order – Relevant court order
- Being an obstacle to construction or for timber requirements in a construction (two trees) – Copy of the building plan approved by the relevant local government.
- In case of serious illness, disaster cash in case of financial need in case of disaster (three trees) – Copies of relevant medical certificates, documents confirming the disaster.

- Field Visit Order from the Divisional Secretary
- Conduct the field visit
- Final Approval from the Divisional Secretary based on the field visit recommendation.
- Permit Fee: Pay a fee of Rs 236.00 per permit issued.
- Submit the permit for signature by the Divisional Secretary or Assistant Divisional Secretary.
- Handover: The applicant signs the permit, and the original permit along with two copies is handed to the relevant Grama Niladhari.
Timber Transportation Permit
Client’s Responsibilities:
- Please complete the first page and the relevant sections to you in the second page of the application form correctly.
- Please list the timber in descending order of the average circumference of the timber logs.
- The Grama Niladhari should take measurements related to the timber list.
- When including round logs in the timber list, it is not necessary to mention those logs separately, it is sufficient to mention the total number of round logs for each class (1 class round logs, 11 class round logs, 111 class round logs).
- The copy of the deed of the land where the trees are situated and the copy of the plan of the land should be certificated by the Grama Niladhari and submitted along with the application.
- If there are any co-owners or other partners, their consent must be expressed and signed consent letters must be submitted before the Grama Niladhari. If deceased, submit the relevant death certificate.
- If the land is undivided, the consent of the other parties must be obtained in writing.
- In case of mortgaged land, written consent must be obtained from the relevant financial institution.
- If you purchased the timber from the landowner, submit a receipt for the appropriate amount. If you received a donation or a gift, submit a letter from the land owner to confirm this.
- If transporting Jackfruit, Ratadel, Palm Oil Tree, Coconut, the original cutting permit must be submitted.

Business Name Registration Permits
Needed documents:
The Application from completed in English (must be certified by a Justice of the Peace or a Commissioner of Oths with a fifty rupee stamp).
The reported obtained from the Grama Niladhari regarding the business location (Grama Niladhari Report).
A true copy of the National Identity Card/ Valid driving license/ valid passport certified by the Grama Niladhari in your resident area.Trade License (issued for the relevant year)
Residence of the business owner/ owners, if in a division outside the Grama Niladhari division where the business is located, residence certificate obtained from the Grama Niladhari of that division.
If it is a partnership, a copy of the partnership agreement (if there is a copy of the partnership agreement)
If it is a partnership, a formal affidavit (certified by a Justice of the Peace or Commissioner of Oths) submitted separately by all partners to substantiate the information stated in the application form.
A copy of the original deed or lease deed registered at the Land Registration Department to prove ownership of the business premises, certified by a notary, (required documents are on the back of the Grama Niladhari Report)
Other documents required depending on the nature of the business (mentioned on the back of the Grama Niladgari Report). If the business owner does not have the qualifications related to the business mentioned in the Grama Niladhari report, a person who has completed the qualifications must sign an affidavit expressing their willingness to work in that business and submit certified copies of the relevant qualification certificates and a certified copy of their identity card.
Charging fees for business registration (this fee system applies to sole proprietorships and partnerships),
- Capital investment up to Rs.299,999/- , for small scale business – Rs.1500/-
- Capital investment from Rs.3000,000/- to Rs.499,999/- – Rs.2500/-
- Capital investment Rs.500,000/- for business above – Rs.3500/-
Late fees – A business must be submitted to the office or registration within one month of commencement. Otherwise, fine of Rs.500/- per month will be levied.
The starting date can be any date within a year and documents must be submitted through the Grama Niladhari to prove it.
The business name amendment fee Rs.3000/-
