Procedure to be followed to access information

Orders - Gazette Extra Ordinary No. 2004/66 dated 3rd February 2017

Request for information can be made to the Information Officer mentioned in the Act using application No. RTI1 in order to obtain information in relation to the Divisional Secretraiat Padukka under the above Act. Submission of this application is not compulsory.

Applying for information

01. Submit application No. RTI1, letter or make a verbal request to the Information Officer for obtaining necessary information and obtain an acknowledgement.

02. You will be informed as expeditiously as possible and in any case within fourteen days whether the information requested by you can/ cannot be provided.

03. In case when it is decided to issue the information requested, you will be informed the charges determined by the Commission, if such charge should be paid. You will be provided with the information requested within fourteen days on payment of a prescribed charge, if it is necessary or free of charge.

04. In case where it is difficult to provide information requested within fourteen days, after paying the prescribed charges, the Information Officer will provide you the information requested within an additional period not exceeding 21 days informing the reasons for the extension of the time frame.

05. Where the request for information concerns the life and personal liberty of the citizen, the response to it shall be made within forty-eight hours of the receipt of the request.

06. If you are not satisfied regarding the following responses you receive for your request for information, kindly note to submit an appeal to the following officer within fourteen days.

  • refusing a request made for informationrefusing a request made for information.
  • refusing access to the information on the ground that such information is exempted from being granted under section 5.
  • non- compliance with time frames specified by this Act.
  • granting of incomplete, misleading or false information.
  • charging an excessive fees.
  • the refusal of the information officer to provide information in the form requested.
  • the citizen requesting having reasonable grounds to believe that information has been deformed, destroyed or misplaced to prevent such citizen from having access to the information.

How to come to the Divisional Secretariat Padukka?

  • From the direction of Padukka to Colombo 125 bus route ,After reaching Padukka town and reaching 50 meters on the Padukka - Horana main road, the Padukka Divisional Secretariat is located.

  • There are 46 Grama Niladhari Divisions and named them according to their relevant numbers .

How to get the business registration?

  • The entrepreneur (owner) of the business should compulsory come with the relevant documents including the National Identity Card.

What are the request documents you have to bring in order to redeem pawning articles and deposited money of the dead person?

  • Death certificate of the deceased 
  • Documents to prove the applicant’s relationship with the dead (Birth Certificates, Marriage Certificates)
  • Affidavit statements to prove that there is no objection to offer the applicant money or goods to the affiant (if there are other members in the family of the deceased)
  • National Identity Card.

What are the necessary documents to change the ownership of the electricity connection?

  • Deed to claim the ownership of the land
  • The letter issued by the House Development Authority or the Urban Council confirming the ownership of the land

How to get the Rs.20,000/-  nutrition allowance for pregnant mothers ?

  • Get an application from the Pre-Childhood Development Officer or the relevant officer of the divisional Secretariat of the maternal clinic you participated belongs to before 3 months from conceiving.

Who can apply the  educational Bursary?

  • Children who go to school are eligible for this bursary if their father is dead or permanently disabled.

How to get the Non governmental Organization?

  • Non-Government Organizations which haven’t exceeded 01 year after its commencement can be registered once they hand over the relevant documents.

can new Samurdhi allowance be accepted?

  • New applications for Samurdhi Allowance won’t be accepted for now. But the appeals are accepted to be considered in the future.

What are the eligibility requirements to get the Elders Allowance?

  • Being more than 70 years of age on the date of application
  • Rs. 3000/- or less family income for month
  • Only one person of the family (either husband or wife) can apply for this program.

How to obtain an original copy of  Birth ,Marriage and death  certificates ?

You can obtain Birth, Marriage and Death certificates from the Divisional Secretariat

  • Rs.120.00 is charged if you know the number and the Date of the relevant certificate.
  • If you don’t know the number and the Date of the relevant certificate it will be charged Rs.250.00 to get the certificate.
  • Above mentioned certificates can be issued by the Padukka divisional Secretariat if only the occurrence of the event was within the Colombo District.

How to register a birth?

  • A normal birth can be registered within 03 months of the occurrence of the birth by the Birth registrar of the relevant Division.
  • Contact the Additional District Registrar and get relevant information to register a birth if it has passed 03 month period of the occurrence of the birth.

How to amend the birth certificate?

Contact the Additional District registrar with relevant documents.

Pension section 

How to change inheritance of the pension to the  widower?

  • Firstly, take the death certificate of the pensioner in order to stop paying the pension.
  • Make necessary arrangements in order to withdraw the excess money from the bank.
  • Then the widower should bring below mentioned documents.
  • The original copy of the death certificate of the pensioner.
  • The original copy of the birth certificate of the widow or widower.
  • The original copy of the marriage certificate.
  • A copy of the widow’s or widower’s bank passbook.
  • A copy of the widow’s or widower’s National Identity Card.
  • Either a copy of the widow and orphans Pension Card or Its number.
  • A color picture of the passport size endorsed by the GramaNiladhari.
  • Photo copies of all aforesaid documents endorsed by the GramaNiladhari.
  • Finally, a relevant officer will check the accuracy of above documents In order to make necessary arrangements to issue the widow and orphans Pension.

Reason for not paying the pension?

  • If the Life certificate for the relevant year of the pensioner is not handed over the pension can be cut down.
  • So the pensioner should get the Life certificate and get the endorsement of the Grama Niladhari. 
  • Once the Life certificate is handed over to the relevant officer, the pension can be updated.

Why wont the pension be paid to the inheritors after the death of the widow ?

    • According to the Act of widow and orphans Pension, there won’t be legal provisions to pay beneficiaries to the inheritors of the owner of the widow and orphans Pension after their death.
